Things I find of interest is of course all the correlation to lab values but more specifically how it talks about the kidney damage.

"A side note about acute kidney injury (AKI): There appears to be a disproportionate number of patients with AKI from COVID-19 compared with typical viral pneumonia. This may suggest another contributing factor, possibly a microangiopathic component."

I am not kidding when I say it is alarming to me the kidney damage from this viral infection.

Also the proposed treatment of Chloroquine and Azythromicn...this maybe effective but can cause complications

"Hydroxychloroquine and thiamine:*These are being recommended on the floors. The data available so far are mixed, but we don't have much. It would be worth considering starting chloroquine or hydroxychloroquine for critically ill patients, depending on availability. Antiviral therapy may be effective primarily early in the disease course, before the development of ARDS. Initiating this in the ED is probably the best bet, but this is not routine practice at this time. Remdesivir is emerging as a frontrunner for antiviral therapy, but it is not widely available at this time. It may end up doing more harm than good, but we will know more with further investigation. Our current dosing on the floors is thiamine 200 mg IV Q12 x 6 days and chloroquine 500 mg BID x 5 days. See below for QTc considerations. Also consider G6PD for patients getting chloroquine; that is why it should be sent with labs at admission.

Azithromycin:*We are not starting this on everyone. Keep in mind that both azithromycin (although mild) and chloroquine are going to prolong the QT, so be careful and aggressively replete Mg and K as needed. The efficacy has yet to be proven."

These medications mess with the heart and need to be carefully administered. Especially in patients with heart issues and this virus starts affecting the heart because if you are not perfusing O2 it causes the heart to work harder.

But the most interesting thing to me is that they present with thrombocytopenia means they have low platelets why?? Some patients over respond to the infection their immune system causes a cytokine storm. This makes them use up all of their platelets and coagulation factors. They are hypercoagulable. Person who had normal platelets are now clotting all over the place. Usually we have negative feedback system that keeps us from doing this but the cytokine storm effectively stops this negative feed back loop. This puts a patient at risk for DIC

Why does this matter? DIC probably one of the most intersting responses of the body to trauma if you ask me.

Disseminated Intravascular Coagulation basically your body starts using up all your available clotting factors to the point you have nothing left and start bleeding internally and you can bleed externally.

Covid-19 had us all fooled, but now we might have finally found its secret.


Eventually, if the patient’s immune system doesn’t fight off the virus in time before their blood oxygen saturation drops too low, ventilator or no ventilator, organs start shutting down. No fuel, no work. The only way to even try to keep them going is max oxygen, even a hyperbaric chamber if one is available on 100% oxygen at multiple atmospheres of pressure, just to give what’s left of their functioning hemoglobin a chance to carry enough o2 to the organs and keep them alive. Yeah we don’t have nearly enough of those chambers, so some fresh red blood cells with normal hemoglobin in the form of a transfusion will have to do.

CO doesn't displace Fe it binds to the iron (Fe). CO has a higher affinity for heme than O2 does. That is why carbon monoxide is so deadly. You can't have a hemoglobin molecule without the iron in the center. And no virus is going to kick iron out of its spot.


Also what he suggests near the end with giving blood transfusions... following his logic of iron overload form all this "misplaced iron"...you would exasperate this by giving a patient a blood transfusion.

Yes you would increase their ability to get oxygen to the tissues for a bit but what would stop the virus from "kicking out the iron" in the blood you just transfused? Thus setting more of it free. Nonsense

I do think the virus could potentially disrupt the process of synthesizing hemoglobin but I can't find any article that currently speak to this. If this were the case a hallmark lab test would be decrease in red blood cells, hemoglobin, and hematocrit. But that is not listed as a hallmark lab result for COVID patients. Lymphopenia is this is a decrease in one of 5 white blood cell types. And decrease in platelets. Or thrombocytopenia.

I do think a hyperbaric chamber could be beneficial maybe... but they are in shorter supply than ventilators.

Some governors called for massive assistance out of fear, and it never reached expectations.

There are some temporary hospitals though that are ceratinly not for COVID19 patients, and this has been detailed in public. It was to help the hospitals concentrate on COVID, while these temporary hospitals took other patients. What is NOT getting a lot of public airtime though, and is certainly happening in the meantime, is the rescue of victims of human trafficking. Almost every press conference, there's a small sentence, or a tiny hint at human trafficking operations.

The way this helps is for those that have been infected, but not symptomatic, or low grade symptoms. They keep those droplets to themselves. But if you are not sick, wearing a mask is really pointless. It can help some people stop touching their face as much. Most people don't even wear a mask the right way. To be truly effective you can't have any air escaping around the mask once you have it on. I use to help fit our techs and phlebs for N95 masks.

I see nurses on TV that are talking to reporters that pull the mask down below their mouth to talk. Well right there if the outside of that mask was infected now you potentially are. Only good it does in mass use is if a person is sick and they cough with a mask on it keeps most of the droplets contained in that mask. What do they do with it when they get home? Use it again the next day? I hope not, but people do not have brains.

The way to have been prepared for this was:

1. To have had testing in place before the virus hit the US that identified infections and quarantine infected persons.

2. Contact tracing to isolate individuals that came in contact with an infected person..

3. Antibody testing, to determine when they had cleared infection and developed immunity.

4. Scrubbing the shit out of every public place better than they are cleaned currently. I laugh at people that just wipe down the grocery cart with a wipe and think it is good to go. It is not. You need to let a disinfectant fluid sit on the surface for a few minutes. Every cleaning fluid or disinfectant will tell you the contact time needed to adequately clean or disinfect a surface. Know the difference between clean and disinfect. Because the two are not one and the same.

5. Education Education Education on how to wash your hands, keep your hands out of your face, how not to be afraid and panic buy TP of all things and yes how to correctly use a mask. But the mask alone would not have allowed us to continue as normal.

But none of that was in place, and it is too late now. There is no win with our current situation not even two months ago because nothing to fight this virus was in place. Sitting around blaming Trump, China, Democrats, Republicans is useless BS.

Do nothing and let natural selection happen, it still would have incited panic when there was death after death. I think most people find this scenario unfathomable. But it has happened in the past and changed normal, just not in our memorable lifetimes. After this it will happen again, hopefully not in our lifetimes.

Do what we are currently doing shutting down the world and you have a another problem that is pretty unfathomable.

Trump is going to try and suspend Habeas Corpus, this means holding people before trial. Where could they be held in the big problem cities? How about some newly prepared "hospital" ships. By the way, what's the next escape route if you can't fly? Water. And the Coast Guard is secretly ALL OVER that right now.

EDIT: New theory for the hospital ships, they will be for the rescued victims of human trafficking. Potentially hundreds of thousands, hopefully, will be freed. Can you imagine what a wake up call this will be in 2 weeks?

I can't find the post on Facebook but in one of the medical groups I follow someone asked what the status was of everyone's hospitals. There were over 300 replies but pretty much everyone was reporting hours being cut. People using PTO to keep a paycheck. Salaries being cut and very few COVID patients.

If I come across it again I will post it.

I will say we had one that had been hospitalized for 4+ weeks. I don't know when that person came in but we get daily updates and that has been going on for 4 weeks. Touch and go for a bit. Thankfully they have gone home. Had a few other cases. But nothing significant or overwhelming. I would say about the same as what I was reading on that post.

This chick got on the local city FB page to rant about her recent quick trip to Wallyworld, and how there were so many people there laughing, having a good time, not wearing facemasks or gloves, buying non-essential items. The thing that i though hilarious about her rant was the fact that she revealed that her excuse for why she had to go to Wallyworld in the first place was to get an ingrediant for her casserole. When i called her out on her 'vital necessity' that she needed to make a trip to Wally World, she doubled down on her necessity by further explaining that it was cream of broccoli that she so desperately needed, and that was the only meal that her daughter wanted. Before all of this took place, due to isolated incidents actual police brutality, in conjunction with widespread rumors and false accusations of bad police, the community support for cops around the country was getting more and more polarized. With stupid crap like this, people being arrested for paddle boarding on an empty shoreline, and other such stupid acts of police enforcing extreme tyrannical rules put in place by local governments, I can only see that polaraization getting bigger, and swaying some from one side of the argument over to the other. This type of asshattery will only create more dislike, distrust, and disrespect from the community for PDs around the country.

Here is an interesting article on using ECMO to treat two patients in severe COVID crisis.

According to this article 80% of patients that end up on ventilators die because their lungs are so damaged they can't handle being on the ventilators. ECMO removes blood, oxygenates the RBCs for you, then returns the oxygenated blood to the body giving the lungs time to heal.

One of the patients was gave a drug that helps pit the breaks on the overactive immune system stopping the cytokine storm.

Don't get me wrong. I absolutely believe China has and continues to cover up information about this outbreak and it does nothing for debunking the man engineered conspiracy theory and everyone wants a "reason" this has happened and they want to put blame on someone.

China should have taken this seriously and not tried to cover it up. And they definitely need to stop trying to limit and control information coming out. They need earnest transparency around this but hey this is China we will not get that. So if they get blamed for this virus being engineered well they deserve it. But I prefer science over conspiracy. I haven't had more time to find more research on this subject but plan to that is another reason I bookmarked the paper.
