I guess I should make an official thread on it, all the pics, video, and explanation together in one place.

It happened at San Tan Mountain Regional Park near Phoenix Arizona. I was riding with my brother in law Terry.
I had taken the wheels off of my Kona to transport it. When I arrived in Arizona I must not have tightened the quick release enough, but it did last 10 miles before coming off.
Im in front of Terry. Then ON THE WAY BACK to the parking lot, maybe 500' away, cruising along at @ 20MPH on some easy doubletrack dirt trail ( :facepalm: ) you can see the video footage of me lifting up over a small divot and the wheel just comes off.
I don't remember the incident at all, in fact I can't remember anything about 5-10 minutes prior, to about 20 minutes after.

So here's the timeline;
We log about 10 miles of riding during the day before the incident, recorded in this video. No, this one does not show the crash:

And just like that, I'm knocked out, unconscious, but very audible. I'm still recording the whole thing and I'm making those disturbing dying sounds, that's why I'm not quite comfortable posting it here yet. Terry says my eyes are open the whole time and he's trying to ask me where I'm hurt but I'm just making those horrible sounds, unresponsive.
He gets right on the task of calling 911 and notifying my wife. This is all heard in the footage as well.
My horrible breathing/gasping ceases within 3 minutes, I begin to "wake up", and sit up on my own soon after. I'm just sitting there dazed and wondering where I am, how I got there, what day it is, what YEAR it is, and most of all what happened. I ask these questions about 10 times EACH for the next 20 minutes until the ambulance shows.
Terry retrieved my front wheel :cool2:
I DECLINED the helicopter transport, fortunately I was coherent enough to overhear that ridiculous discussion so I spoke up and said NO. I settled for the ambulance even though I didn't want that either but I did it out of respect for Terry and my wife.

From watching the video, I think the crash happened here, you can see how close we were to the trail head on the way back, and how easy it was for the paramedics to reach me.

I spent 3 hours in the hospital, they stuck me 5 times because I was dehydrated. I don't even know what the hell the IVs were for at this point. They needed to just keep an eye on me because of possible brain trauma.
When I was released that night my wife had to wake me up a few times in the night to make sure I was ok.
The injuries so far; I got a huge concussion but at the moment it seems there is not long term damage. I've got a sore neck, left shoulder, scrapes in about 6 places, and no broken bones as indicated by xray. I'll get an MRI next week if I'm still having pain in my shoulder.
My helmet ABSOLUTELY SAVED MY LIFE. Not just from further injury, but literally absolutely hands down saved my life. With the helmet on, I knocked my head so hard that I didn't know what year it was. Think about it.
I was extremely lucky, and although I constantly hear it, nobody realizes it more than me. So many things could have been worse. I'm literally walking away from this thing with a few scrapes. I even kept my teeth!
