HELP, where do I find the form for taking Scouts fishing w/o a liscense????

I have used it before, but cannot find it on DWR's site.

In the past I have gone to that site and there is a form you fill out and then you can take kids fishing who dont have a liscense. I need it for tonight. Help please.

PS the DWR offices are closed today.......
