There was a blip on the news morning of some accident in Zion where a pulley was used wrong? Is this recent or just a follow up on previous accident?

Yeah, follow up to the accident on Touchstone Wall last fall.

Lead climber rigged the haul with a Petzl Pro Traxion self capture pulley, but, failed to close the device completely and didn't clip a back up biner to the botton of the device (which would prevent it from popping open). The guy jugging the pitch (along with a 105 lb pig) got onto the rope, the rope rigged through the pro traxion unloaded, dropping him 15 feet, and his jugs cut the rope.

Lessons to be learned the hard way, to be sure.

Sad deal.

105 pound haul bag aka "a pig". Wall climbers commonly refer to their haul bags as "pigs". These guys were spending a night on the wall, so, had a heavy load of stuff they were hauling up the wall with them. The guy that died was ascending the loaded haul line.

A "jug" is a mechanical ascending device like a Jumar, Petzl handled ascender, etc.
